Dull, scaling, blotchy skin with wrinkles can make patients look older than they feel or really are. Along with the skin care staff at our Beaura Spa, we aim to brighten the patient's complexion, smooth the skin, revitalize the face, equalize the color, and improve fine wrinkles. Maintaining healthy skin requires ongoing attention.
The first step on the road to healthy skin is a thorough skin evaluation to identify special problems. We specifically look for loss of skin vitality, discolorations, skin roughness, large pores, and different types of wrinkles.
Crepe-paper wrinkles are fine wrinkles usually on the cheeks from sun exposure. Dynamic wrinkles (also known as "smile lines" or "frown lines") result from repeated facial expressions forming creases and eventually wrinkles. These are narrow depressions in the skin most commonly seen in the forehead, between the eyebrows, crow's feet, and vertical wrinkles around the lips. Skin fold wrinkles occur as the cheeks sag with aging. These are most commonly seen from the corner of the nose to the corner of the mouth (called "nasolabial folds") and from the corner of the mouth downward (called "marionette lines").
Fine lines and textural problems of the skin are addressed non-surgically. Improvement in the quality of skin color and texture can be initiated by the use of various creams prescribed by our skin care team at our Beaura Spa. Acid peels, intense pulse light (IPL), and microdermabrasion can augment this initial treatment.
Crepe-paper wrinkles are significantly improved by physician-supervised acid peels. Dynamic wrinkles in the forehead, between the brows, and in the crow's feet around the eyes are effectively treated with Botox® Cosmetic This is a purified protein injected into the wrinkle lines which "relaxes" the underlying muscles causing the wrinkles to soften or disappear. Botox Cosmetic injections take 5 to 10 minutes to perform and become effective within 24 to 48 hours. They will usually last 3 to 6 months and seem to last longer and longer with repeated injections. Dynamic wrinkles of the lips are most effectively treated by resurfacing the area with dermabrasion, laser resurfacing, or chemical peel.
Skin fold wrinkles at the nasolabial folds and marionette lines are best treated with a facelift. A facelift can tighten facial skin, improve heavy skin folds, and soften associated wrinkles.
A more modest improvement of these folds can be achieved by injecting them with "filler substances" to try to raise the wrinkle to the level of the surrounding skin, making it less noticeable. Collagen has been the "gold standard" filler substance for many years. It requires skin testing for allergy and loses its effectiveness within several months. Restylane, Hylaform and Captique are three commercially available forms of hyualuronic acid, a naturally occurring substance. Injection into the folds or wrinkles takes 10 to 15 minutes to perform after the application of a topical anesthetic and ice. No skin testing is required. They seem to last 6 to 12 months before requiring repeat injection.
Radiesse (hydroxyappetite), formally known as Radiance, is a synthetically derived compound which has been used in Europe as a filler substance for a number of years. It is injected into folds and wrinkles like Restylane, Hylaform or Captique. Our colleagues in Europe suggest it lasts two to three years before requiring repeat injection. Skin testing is not required.
Fat injections were used more frequently as a filler substance before the arrival of Restylane, Hylaform, Captique and Radiesse. It requires "harvesting" or removal of fat from a donor site, "purification" of the fat and then injection into the area to be corrected. Prominent wrinkles or folds requiring more volume than can be easily delivered by the commercially-available fillers are usually treated with fat injections. Common treatment sites include the nasolabial folds and marionette lines on the face and injection into the dorsum of aging hands to replace lost volume. This latter procedure is known as a "hand lift." Fat injections can also be used to "fill" the area beneath depressed scars to improve their appearance.
